The Resident may withdraw from the Residency Training Program if he/she has a valid reason provided that the consent of the Education Committee, OMSB, and sponsor have been obtained.

If the Resident who has withdrawn wishes to rejoin at OMSB, he/she should be evaluated by the Education Committee to determine the possibility of accepting him/her into OMSB’s residency training program and the appropriate training level.

Withdrawal Application Process

  1. The Trainee submits a Withdrawal Form to the OMSB Admission & Registration Section
  2. The OMSB Admissions & Registration Section processes the request and refers it to the Education Committee for their opinion/resolution
  3.  If the withdrawal process is still ongoing, following the Education Committee’s involvement, then a Referral must be made to the Performance and Wellness Unit
  4. The Report generated by the Performance and Wellness Unit is sent to the Education Committee for their final recommendation
  5.  The Education Committee sends an official letter of approval to OMSB to process the Trainee’s withdrawal
  6. The Trainee must then obtain approval from Sponsor
  7. The Trainee must continue training in OMSB until the Cancellation of the Release
  8. The Trainee must fill in an OMSB Clearance Form and submit it to the Admission and Registration Section
  9. The Trainee must complete any other requirements recommended by OMSB
  10. The Trainee must report back to the sponsor directly after withdrawal from OMSB
